Chaos Axes can primarily be found in the Fissure of Woe or the chests that spawn during the Kilroy Stonekin quest. Sometimes they can be found in Underworld chests. Inscribable Chaos Axes can be found from reward chest after completing all the Fissure of Woe quests.
