The Reaper of the Twin Serpent Mountains is a servant of Grenth, who governs the Twin Serpent Mountains of the Underworld. He can be found by Grenth's Statue in the Twin Serpent Mountains. For him to appear you have to destroy the Terrorweb Dryders around the statue. He can teleport the party to the other Reapers of the Underworld, as soon as they have been freed (see quest Restoring Grenth's Monuments).
Take care not to let any monsters come near the Reaper. If he dies, your party will be kicked out of the Underworld.
Note: Freeing this reaper is somewhat tricky as the Dryders guarding the statue have Obsidian Behemoths behind them. Make sure to kill the behemoths along with the Dryders or kill them first. Killing the Dryders first will cause the Reaper to appear and get attacked by the Behemoths. However, this is not so bad, as the Reaper is usually smart enough to stay away from them (and, by extension, their traps). If they attack him, just heal him and kill them.

Grenth was so angered by these two mortals, that instead of simply granting them the sweet peace of death, he turned them into twin serpents and forced them to serve him in the Rift for the rest of eternity. This mountain stands as a reminder to all those who serve Grenth. Be careful that you do not incur such a punishment."
