The damage is dealt to target foe as well as nearby foes, but only the hexed enemy is interrupted.
If the target is hexed with both Clumsiness and Wandering Eye, both will deal the damage. This means only one attack interrupt will occur, but it creates a large, if avoidable, spike.
If both Wandering Eye and Ineptitude are on a creature, they won't trigger on the same attack. Wandering Eye will interrupt the attack before Ineptitude triggers.
